Once you have scheduled your appointment with Kate, she will send you an email with paperwork to be submitted electronically. Once you have completed the electronic email, she will be informed and will review the paperwork before session. However, if you would prefer to fill it out the old fashion way, please complete these forms and bring them with you to your first session.
